There are two fields in this form what can be customized:
url - change YourUrl with http://yoursite/yourpage.
title - change YourTitle with title of your site.
With this two fields, script will produce link back to your site.
Make a test. On bottom of search results page you will see link back
to your site (back to YourTitle).
